Abstract Background Dislocations of the elbow are second most common dislocations humeral joints following shoulder. Besides numerous possible concomitant injuries collateral ligaments or extensor flexor apparatus, an accompanying disruption brachial artery is a rare occurrence. In following, such case presented and discussed. Method A 70-year-old woman sustained closed posterior dislocation with due to fall in domestic environment. Pulselessness radial led computed tomography angiography being performed, which confirmed diagnosis. Direct operative vascular reconstruction vein insert was carried out. Due strong swelling soft tissue, other examinations could not be performed initially. redislocation few days later stabilization joint. Results The final consultation 4 months postoperatively showed stable, centered joint normal perfusion affected arm. function good range motion 0/0/110° extension/flexion. Conclusion An complex injury. accurate clinical examination important should repeated first after Vascular immediately. persistent instability, indicated may supported by hinged external fixator.
